HOME CARE NURSE
APPLICATIONS ARE INVITED FOR PART-TIME (0.5 FTE) HOME CARE NURSE FOR BLACK LAKE, SASK. GENERAL ACCOUNTABILITY
Under the supervision of the Executive Director of Primary Care Services, the Home Care Nurse [HCN] combines the knowledge of holistic health assessment, clinical nursing skills, administration skills and supervisory skills to effectively plan, assess and coordinate client care. The role of the HCN is to coordinate all services received by the client in such a way as to meet the needs of the client effectively while maximizing client independence with family and community support. The HCN may provide supervision to other home care staff including Home Health Aides, Homemakers, and Home Maintenance Workers.
DUTIES AND RESPONSIBILITIES
- Within the HCN scope of practice, the nurse is responsible for the provision of effective home care program delivery
- The HCN is ultimately responsible for coordination of client care, monitoring the quality of services delivered, and, where this responsibility is delegated to him/her, clinical supervision of other Home Care staff including Home Health aides, Homemakers, and Home Maintenance Workers.
- The HCN is involved in record keeping, data collection, program monitoring, reporting and program evaluation.
- The HCN assists in program management, monitoring, evaluation and on-going development of the HCCS program
- Perform other duties, as required
QUALIFICATIONS
- Diploma or Bachelor’s degree in nursing from a recognized school of nursing
- Eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) or Saskatchewan Licensed Practical Nurse (SLPN)
- Previous home care or community health experience
- Experience in Northern Saskatchewan and First Nation locations considered an asset
- Cultural awareness and sensitivity to the dene culture and first nations communities
- Educational/theoretical participation in Community Health/Public Health/Home Care Nursing
- Valid Class 5 Driver's License
