Primary Focus
Reporting directly to the Operations Supervisor / Director of Developmental Services Ontario Eastern Region, the Matching / Linking Coordinator is responsible for ensuring the completion of all steps related to the matching / linking of eligible adults with a developmental disability to available MCCSS funded services. The Matching / Linking Coordinator ensures that service delivery is provided in accordance with the legislation, regulations, standards and Ministry directives. The Matching / Linking Coordinator is responsible to work collaboratively with local, regional and provincial community partners and service groups, and provides information on the role of Developmental Services Ontario Eastern Region.

Gathers and verifies resource declarations.

Analysis listings of prioritized individuals as potential matching candidates.

Provides recommendations on suitable placement candidates to declaring service providers and manages agency responses.

Informs individual/candidate and their support network of agency consideration.

Links candidates with potential agency matched.

Documents all steps of the matching/linking process.

Ensures that ministry established timelines are adhered to.

Provides information regarding available service vacancies to community Urgent Mechanisms.

Collaborates with MCCSS, community planning tables, service resolution groups, and case management services across the region, providing accurate and current data on available vacancies.

Prepares monthly reports.

Responds to inquiries from agencies regarding vacancies.

Maintains data integrity for both, agency and client files.

Participates in regular performance supervisions with DSOER’s Operational Supervisor (direct report).

Provides recommendations regarding complex and/or ongoing concerns impacting the matching and linking process.

Adheres to all mandatory agency policies and procedures.

Performs other duties as assigned.

A minimum of 5 years of relevant experience.

A Bachelor’s degree in Social Work, Social Services or Community Services or related field.

Excellent communication and interpersonal skills.

Must have strong analytical and reasoning skills.

Strong understanding of requirements for coordination of services across multi-agencies and Ministries of Health, Social Services and Education.

An ability to deal with all levels of organizations.

Knowledge of the services available in the eastern region for adults with developmental disabilities.

Demonstrates professionalism in managing complex and/or sensitive cases, and has excellent communication skills.

Knowledge of the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act, 2008 is a requirement.

Advanced fluency in both official languages (French and English) in oral, written and comprehension functions.

Excellent organizational and time management skills.

Skilled at working in a fast-paced and, at times, demanding environment.

Proficient in the use of Internet, databases, Microsoft Office Suite including Word, Excel, PowerPoint, Outlook

Must have a valid driver’s license and access to own vehicle for business travel.

The following Paid Time Off (PTO) policies are offered to all permanent and term employees:

Three (3) weeks (8.75 hours per months worked) vacation and sick time

Two (2) float days to be used throughout the fiscal year

Five (5) days for personal time to be used throughout the fiscal year

Paid time off between Dec 25 and Jan 1
Work is completed in a virtual and in -person hybrid model. Staff of SCS are expected to be available to work within any DSOER office and to have designated, private area available in their home to work. SCS employees are responsible to have appropriate level of internet access in their homes to complete work virtually. DSOER staff can be required to attend meetings locally, regionally and/or provincially.
